Annual Mass Enrollment
Use this online form to enroll your loved ones—living or deceased—with an Annual Enrollment in our Mass Association. This online form processes your donation using PayPal. You don’t need a PayPal account to use Pay Pal. Once you have enrolled your loved ones we will ship the enrollment folder to the address you provide. Please allow two weeks for delivery.
